Care Continuum, Inc. is an international managed care implementation company that began operations in 1995. Since its foundation, its primary focus has been to develop working relationships with:

• managed care Health Plans
• physician networks
• government regulatory and national accrediting agencies.
  

Client engagements

These involve:

• developing medical management and regulatory compliance programmes, processes, policies and procedures;
• providing interim medical and compliance department management while assisting in the hiring and training of staff;
• completion and submission of state and federal managed healthcare applications;
• on-site surveys with corrective action recommendations for state and federal managed healthcare programmes.
  

Specialist consultants

Care Continuum’s consultants have led the development of case management systems both in the United States and in the UK: working with Conrane on the Castlefields case management development initiative in Runcorn, UK. They provide specialist consultants in the field of case management from senior nursing and social work backgrounds with both project development and training skills.
