Case Management Development

The key to the successful implementation of case management is to ensure all components of an effective model are in place. Conrane Consulting and its partners have developed tried and tested approaches which we adapt to local requirements. We can deliver each component individually, or as part of an integrated package. The key elements of our programme include:

Training Community Matrons

Our bespoke training programme includes:

  • Two taught modules that have been designed to deliver the domains of competence required by Community Matrons;
  • Expert tutors drawn from medical, nursing, pharmacy and allied health backgrounds;
  • One-on-one mentoring /or action learning to convert learning into practice.

The programme is completed by assessment of competence in the workplace. Training is delivered in cohorts of up to 12 students, and each Community Matron dedicates 20-30 days to the programme over a 4- to 5-month period.

Systems Development: Tools to support the Community Matron function

We have a developed suite of evidence-based and user-friendly tools that support the Community Matron function. These tools include:

  • Predictive modeling and population risk profiling
    These identify high-risk patients with complex needs who fulfill the criteria for case management. Wider population profiling also finds patients at all levels in the ‘Kaiser triangle’ and clinically informs local resource allocation, service planning and commissioning.

  • Patient assessment and care planning
    Specialist and evidence-based tools that are integrated into the local single assessment process.
  • Evidence-based management guidelines
    Which the staff are trained and empowered to use in their practice.
  • Programme evaluation
    This includes a user-friendly programme that enables the Community Matrons to monitor and report on their own effectiveness.
  • Change management
    We work with you to develop local systems that ensure seamless working between the Community Matrons and key link services such as primary care, social services and the local acute hospitals. We also lead stakeholder workshops, and social marketing strategies: including developing each Community Matron into being an effective advocate for their services within the local health community.
