Stakeholder Workshop
A key stakeholder event is run at the inception of the programme to ensure clear and effective communication of the proposed changes to all organisations and agencies that will be involved in the programme. A clear explanation of the model and agreed tools and frameworks will be given. The event will undertake a baseline assessment of work-to-date in the Primary Care Trust and identify local change management issues. It will also task a ‘local implementation group’ supported by Conrane consultants in key work streams proposed as:
- Develop local protocols with key link services
A key critical success factor will be speed of response from other services. Each Local Health Economy/Primary Care Trust has its own network of health, social and wider community services. Hence local referral mechanisms and prioritisation criteria may need to be reviewed and amended to ensure turnaround times synergise with the case management process.
- Engaging GP practices
Engaging primary care is essential but sometimes challenging. These meetings are designed to develop the essential links to individual practices. They set out the aims and scope of the service, discuss a draft protocol, identify specific logistical issues in the given practice. We would conduct the first round of meetings alongside the local project lead who will then lead the remaining meetings using our format and materials for subsequent waves within the Primary Care Trust.
- Local systems requirement
Data capture and reporting requirements will need to be implemented at local level. Conrane and our partners at Imperial College London would build local capacity, including mechanisms for interpreting, delivering, monitoring and evaluation.
- Local service audit
We will lead a review of the current service network in long-term conditions to identify any key gaps and set development and redesign priorities.
- In-reach to hospital
Appropriate mechanisms for coordinated working with local hospitals will be established including triggers and red flags.
- Further roll-out and capacity building
We will support planning of and further roll-out of your local programme.
