Our Partnership Network

Conrane has led the implementation of care coordination –  such as the Johns Hopkins Guided Care Model –  into the NHS.  This was first established in Castlefields Health Centre in 2000 with Dr David Colin-Thome and subsequently in Surrey Sussex and NW London.  We have also worked in collaboration with Johns Hopkins to bring the ACG system to the NHS since 2005 and continue to partner with ACG International to deploy and develop this system into practice.

Johns Hopkins Bloomberg School of Public Health is the home of the ACG risk stratification and predictive modelling system, the Guided Care long-term condition approach  and the development and implementation of hospital at home in the US managed care sector.   Over the last two years, Johns Hopkins have collaborated with Kaiser Permanente in Washington DC and Baltimore to implement Guided Care in the HMO’s local network.  

Johns Hopkins collaborates with Conrane on Guided Care implementation in the NHS and alongside our informatics partners for ACGs.
